Benefit Choice period nearing end
All employees should have received their Benefit Choice Options booklet in the mail. The booklet outlines the changes for fiscal year 2006. The Benefit Choice period is May 1-31. Changes made during this time will be effective on July 1.
Visit the Benefit Choice Web site for more information. The site offers the Benefit Choice Options booklet, as well as the election form, rate sheet, FAQ and a premium calculation worksheet.
The premium calculation worksheet allows to you do "what-if" analysis by making different choices. The worksheet calculates premiums based on your selections and gives you a monthly out of pocket cost for your selections.
The Web site also provides information about the new vision provider EyeMed. The current provider network is posted and will be updated as new providers are added.
You will also find information about the new prescription benefit manager, Medco Health Solutions, for employees enrolled in Quality Care Health Plans and HealthLink OAP. The maintenance, specialty and preferred drug lists are posted. The pre-announcement letter and the retail pharmacy listing will be available soon.
If you elect to make changes during benefits choice, the election form, provided in your booklet or on the web, will need to be completed and returned to Employee Benefits, 805 S. Elizabeth St., MC 6520. All forms must be received in Employee Benefits by 4:30 p.m. on Tuesday, May 31. If you choose not to make changes, simply do nothing.
